Task 3: Written analysis of interprofessional care and self-management plan

Weightage: 50%

Due date: 21st September 2023 at 1700 hrs via Moodle drop box.

Word count: 2000word +/- 10% (The intext citations are included in the wordcount)

To write your assignment, you are required to develop your own customised patient scenario. Your patient must have two (2) of the nine chronic health conditions outlined by the Australian National Health Priorities.

Using the scenario that you have developed; your assignment will include the following.

  1. An introduction: A brief description of your patient, including their age and diagnosis (not pathophysiology of diseases). An overview of the main points to be covered in the assignment, and a summary (brief) of the 2 (two) SMART goals for your patient and their chronic health condition.
  2. A critical analysis of the self-management framework with relevance and clearly linked to your patient and their health issues.
  3. A critical analysis of interprofessional care with relevance and link to your patient and their health issues. Important: In this section, you must:
    1. Identify 2 (two) other appropriate health professionals for your patient and explain how they will assist your patient in their ongoing chronic disease management.
    1. Identify and discuss 2 (two) key attributes a RN must possess for effective collaboration amongst other health professionals.
  4. You are to design a self-management plan for your patient that clearly identifies your two SMART goals – please see below for a format example
    1. For each SMART goal, you are to include 3 (three) patient-guided interventions. One (1) of the three interventions for each goal must include the use of the identified health professionals in part 3. Each intervention must have brief justification (10 words or less)
    1. Each intervention – must have an appropriate and relevant supporting reference.
