Purpose of assessment task:

This task will enable you to demonstrate application of the nursing process: assessment, problem identification, planning, implementation, and evaluation (ULO1) by developing a nursing care plan for a patient Careplan for Cloud experiencing acute episodic illness.

This task will also enable you to explain the underlying pathophysiology of the acute illness/es experienced by the patient in the care plan for a cloud provided scenario to inform evidence-based nursing care (ULO2).

Case Scenario:

You are a student nurse working on an acute medical ward. You are working with a registered nurse and are allocated 4 patients. One of those patients is Ms. Alina Santiago.

Alina was admitted to the ward overnight after presenting to the emergency department (ED) with increasing shortness of breath and a productive cough.Alina is a 27-year-old female, with a past medical history of anemia and has recently completed a course of antibiotics for an upper respiratory tract infection. Alina has been refusing oral fluids, states she is not thirsty, and is feeling fatigued.

Alina has an allergy to penicillin and takes Ferro-grad C 325mg/500mg daily.

Following handover, you complete a set of vital signs on Alina.

  2. Risk of fluid volume deficit.
  3. Ineffective cardiac output.
  4. Impaired gas exchange.

Your Task:

  1. Describe four (4) additional priority patient assessments, including a rationale, that you will need to undertake to safely plan your care for Alina.
  2. Re-construct each of the four (4) patient problems to include the relevant aetiology, defining characteristics Careplan for Cloud &/or risk factors associated with each problem.
  3. Prioritise the four (4) patient problems and identify an appropriate goal of care the includes expected measurable parameters and timelines.
  4. a) Identify two (2) evidence-based nursing interventions for each of the four
    • (4) patient problems.
    • b) Provide a rationale for each of the identified nursing interventions, including an explanation of the underlying physiology and/or pathophysiology.
  5. Describe how you will evaluate the effectiveness of the nursing interventions identified, with reference to the patient’s expected goals of care.
