Annotated Bibliography: In their prospective randomised controlled study, Sang and his colleagues investigated the effectiveness and safety of various prophylaxis combinations in preventing VTE after gynaecologic surgery in patients with varying risk levels. With a sample of 625 women drawn from five referral hospitals in Beijing, the researchers randomised the subjects into four groups: Group A received GCS alone; Group B received GCS and LMWH therapy (dalteparin); Group C received GCS and IPC; and Group D received GCS, IPC and LMWH therapy.
Venous blood tests were performed 7 days prior to and 3 days after surgery to measure haemoglobin, platelet, and coagulation levels. VTE was also assessed a week to and 3 days after surgery using ultrasound imaging and CT angiography. The researchers found that the highest prevalence of DVT was in Group A (8.8%), with Groups C, B and D having incidences of 5.2%, 3.8% and 2.6%, respectively. In other words, VTE incidence was lower in groups treated with LMWH therapy plus another intervention (e.g., GCS or IPC) compared to groups that received mechanical prophylaxis alone.
This suggests two things: 1) a combination of mechanical and chemical prophylaxis is more effective in VTE prevention after surgery compared to mechanical or chemical prophylaxis alone; 2) chemical prophylaxis is more effective in VTE prevention than mechanical prophylaxis.
